Thanky you for the great explanation. I cant lock the doors on my truck, either i have the wrong key, or they are really crusty. And my handles, and dorrs are shot and rotted. Saturday night i am going out and buying a set of used doors, a tailgate, and nerf bars for 100 bucks of craigslist. So i am going to put the kodiak handles in those doors after i get them painted..
I need some clarification on how the lock cylinder comes out of the handle.
I have been plying around with mine tryin to figure it out, but I can't
I searched all over, and i could not find a single Kodiak or Topkick that was a quad cab, they were all regular cab. Have you opened up your back doors yet and seen if the linkage will bolt in? Use one of your front handles and just try it.. I think overall the mechanics of the front and rear doors are the same, their structure might be a little different, but give her a try.

I have been looking at a few 4 door Kodiak and HD trucks When the dealer runs the VIN they all come back as 2 door trucks. I think some carrage shop must have custom made the 4 doors. Another thing is on all the 4door trucks I have seen the rear doors have the lock cylinder in them also. I think if GM did it there would be a special handle without them.
I am just going to get another set of fronts for the rear doors, at some point.
