Engine doesn't idle smooth

Well I don't know I have been fighting this issue for ten years I have to believe it is a simple problem the challenge is putting the finger to it
 
Smoke? What color? These engines may smoke a little white or black, but, stock should not be a fog. Good clue here And Not normal.

Possible the cat converter is clogged or broken up and clogging the exhaust. You can drop the crossover or downpipe to open up the exhaust. It doesn't take much to ruin them.

Do you have a scanner? How did you set the timing on a new or different pump?

How many miles on the engine? Timing chain streached out or ate a keyway?

5 gal pail is at the lift pump. I fought a biodiesel softened hose that kinked shut now and then for a couple years.

Another thoght is to shoot the manifold with an IR temp gun see if one cylinder is "cold" aka not fireing.
 
The timing I did time last the second one I had shop put it in they timed every to the book 6000miles did same thing he scanned it before he started and agreed the pump was bad
