I would agree that you should let it run for a bit at std idle to put some heat into the heads. That and let the oil warm a bit first.
My 96 would sometimes run a bit odd if I tried to drive before warmed up.
The 1070 RPM speed is all that you should need/use IMHO.

The Dmax nozzles are not the problem. While there is a "pop" pressure, the fuel rail is isolated from the nozzle by the solenoid so if there is a leak this likely where the problem is.
In fairness to the Dealer Svc Mgr the GM warranty policy on this emissions stuff os a convoluted mess. IIRC the warranty documentation says it is not, but the service procedure does. I'd hate to have to hunt around to find contradictory info myself...

Have them update the ECM programming. There is an update for EGR motor fast cleaning. Plates are not necessary with EGR shut off unless you are going racing. Any respectable stock transmission code will not "blow open" the EGR valve.

I've been tempted to try HID's myself. Seems that they have really come down in price. I wanted them on my 2002 as the housings/lenses suck compared to the 2003+ but now my 2002 doesn't see much road duty so it's not such an issue.

Actually, the Juice doesn't alter pressure, it does pulse, BUT since the pressure signal goes in and comes out of the harness, it could get "eaten" by a bad connection etc. Like stated above leave it off and see if it clears up.
Yes it's me. I'd suggest you ask the dealer if the ECM updates were loaded. EFI Live or a Tech 2 can check the OS. This would be the dead giveaway. The new OS was just released a couple of months ago if memory serves.

The regulator is on the pump and generally either works fine or lopes/surges.
A simple shim will increase the relief valve dump point, but typically it's a flow deficiency that beats up on the valve so I might suggest adding some "push" on the fuel in the form of a lift pump.
