A couple of questions.
Seems that this is a 2002 engine. Found diamond in the walley.

Real hard running sound at startup for a few seconds. What can cause this?

Nr 9 resistor in pmd.
Found a (what I think is) a boost fooler. Do not get any codes/engine light. Use manual wastegate spring.
9 psi.
Vacuum pump don't work. Egr vacuum line is of and same with boost vacuum.

But shouldn't it throw codes? I think it runs really good. No smoke.

But the sound at startup is deafening. 🤣
Uses fuel like a champ. 11 miles pr gallon. Litle high in a tahoe

Could be air intrusion into your fuel system while sitting...evident on start up by rough idle. Can't speak to your modifications but, if vac pump is inop and their are no codes then perhaps there has been some kind of ECM reprogramming done or someone removed the SES bulb from the instrument panel.

If you can post a video to YouTube, bitchute, etc., then post a link here.
It is normal for ugly start in cold weather for a second or two. With video we can say if it is normal or something is wrong.

Using a mechanical boost controller instead of vacuum will normally lower mpg. Burns more fuel for the gain of power. If not set up correctly it will burn a lot more fuel with barely any power gain.
Boost fooler will eliminate a lot of codes that would otherwise pop up- that is its job.

I will make a video. Light in dash works. I know this because before I made a mechanical wastegate it threw codes on under or no boost .
Light is ok.
Starts easy. Even - 2 degrees Celsius. It runs hard for 10-60 seconds. Like cold start system. Advanced timing says 14-16 degrees on obd2 reader. Desired is following readout on injection timing.
See 0.7 on timing that are within parameters.

Ok. Reason I ask is it might be perfectly fine. This is a unique sounding and rather loud engine. Working in fleets with 6.5 engines, often new employees would come to us with reports of something wrong and they just never heard 6.5 before.
Because it was in Las Vegas where the cold part of our winter is only a few weeks long- people would drive the truck from when they hire on in March, then come with the complaint in end of November when it finally gets cold enough to make the cold 6.5 rattle.

Really make the Youtube video

I don’t hear anything I would worry about.
I agree that he lifters are leaking out their oil and taking a couple seconds to pump up with oil. And the rest of the system is starting dry of oil so there is that sound.
If you really want you could put in a precharger to help with that, but questionable if it is worth the expense on a high mileage engine to pay for itself.

And yes it is cranking slowly. Remove and clean battery cables, load test batteries while disconnected. If that is the starter wearing out, replace it with the powermaster 9052- they make a huge difference.

I have one valve lifter that makes a tick sound. Hear it more when hot.
Changed oil 3 times and did a qmi clean of inside engine. Got better for a while but back on now.
Afraid to drive it because of that lifter. But sounds i talk about is general hard running until cylinders get a little warmed up
