T-Case Tailshaft Play.

Hello all, my truck has been leaking transmission fluid from the t-case output seal for almost a year. I suspected the seal to be going bad, so ive just kept topping the system off. Last time i was under there i greased the driveshaft u-joints and realized that there was some play in the yolk coming out of the t-case! When you push the driveshaft up and down you can see the seal flex. What parts do i need to fix this? I.E. bearings inside t-case? Can possibly provide a video tomorrow. Thanks!
