250k miles, could just be worn out but sounds like there is a underlying issue. Also says it comes with 2 pmd's so he isn't a completely uninformed individual.

What do you guys think? Bad lift pump/ops? Air leak under the intake? Ignition issue? Bad ECM?

The plan on a truck like this would be get it running good and maybe fix the paint, definitely fix the seat. Then sell and try to make a few pennies after ATT, Diamond Eye, Kojo, and Leroy Diesel ran up a bill. DD until It sells.
 
Has fairly high miles on it. Do a compression test.
Could be a simple fix, worst case a new IP (assuming engine/trans are good)
Buy it for $500-600 then if you have to put at worst case $2000 into it your still good.
The high miles would concern me the most.
 
Yeah, low compression has been a thought but the last one I called on was boasting a new GM Crate Engine and starts if you spray enough ether in it. That sounds like a black hole for parts more than a truck that has had maintenance done but has a starting issue.
 
Compression test, paying attenetion to broken crank scenario.
jack up rear axle and check for excessive backlash. If the transmission smells or is dark use it a barganing chip a out the unkown.
I would think you could make some $ on it.
 
If I needed it, I would setup a time and show up with your trailer hooked up, I would start around 500$ and would not go past 625$... and be ready to drive off without the truck, I wouldn't let him know you know some about the motor, non of his business, sounds like he is ready to get rid of it and already knows what's wrong and how many $$$ or so he thinks, get it cheap is the only way I'd load it up & haul it home...
 
Presuming you get the price down some more, use it for odd light duty jobs while you figure out what it needs for getting to a healthy-ish condition. If you learn that it is an unrecoverable money pit and decide to dump it, likely can get the money back in parts / scrap.
 
Looks solid enough to get an optimizer and good paint and keep as a DD or optimizer my 93 and give it's 599 a refresh and drop in this truck (block condition pending)
