hard start/stalls when put in gear after sitting a little while after warming up

Basic problem: the truck cranks really long and will only start with the pedal all the way down when its hot. Also has trouble idling (need to really slowly release the pedal or it will die) and it dies instantly when dropped into gear. This only happens after it has been warmed up and then sat a short while. Cold its fine

The truck normally runs perfect even after a long drive and starts and runs fine when cold. Also it seems like if its turned off and restarted immediately when hot its fine, something seems to heat soak to cause the problem because it only happens after its sat a little while and I go to start it up.

The truck is a 95 with a DS4 pump so it doesn't seem like its the typical hot start problem that DB2 pumps have (I have also tried dumping water on it, that used to work with my old truck with a DB2)

Things I have tried:

- replacing PMD with a few spares I have when it is stuck in this condition (PMD is almost new and remote mounted under the bumper on a heat sink too)- no change
- cracking valve on top of fuel filter (when I can actually got it to idle in this condition). -Fuel sprays out
- replacing fuel shutoff solenoid when stuck in this condition - no change
- putting T case in neutral - didn't stall when put in gear, can't get out of neutral without turning motor off though or T case grinds
- disconnecting battery for over 30 seconds -no change
- disconnecting advance solenoid on side of pump -no change
- dumping a couple of gallons right on the pump -no change

Other things:

- after idling a while it seems like sometimes it will eventually snap out of it and I can put it in gear and drive normal
- leaving truck for a few hours and it always fires right up and goes
- when in this funny state if I increase RPMs I get a puff of greyish sort of smoke, maybe unburnt fuel and it stumbles
- didn't see the check engine light come on when it was idling in this bad state

I am trying to figure out what could possibly be getting stuck, a sensor somewhere?

Something else I will try is to clear the codes, go for a short drive and read them (to find out what codes are always there since I have a turbomaster and the EGR system isn't exactly working I always have some codes), then I will let it get into this state and see if a new code popped up
