Erratic throttle and black smoke

Any ideas? I have a 2000 k2500 with a 6.5 turbo diesel. 245km on it. Runs a bit rough. Starts good. Has an erratic throttle and thick black smoke on acceleration. Can't hold steady throttle. Swapped out pmd module with a known good one. Repaired numerous grounds that were bad or missing. Any cle what's next?

Are there any codes? Try slowing raising engine to 2000 rpm and holding it there for a few seconds then slowing dropping it. this is an indicator of IP health if it can or can't be done.
 
You are looking at a new IP. The first time it goes runaway on start up on you will startle the shit out of you, just turn the key off to kill the motor. But replace the IP ASAP! The life expectancy of an IP is 150,000 miles. Keep running with an IP that's going out can also take out your tranny with it (trust me) because of the "surging" at cruise.

Fixed it
 
And the usual cause is a weak lift pump or other fuel supply issue that caused extra stress on the IP to wear it out so go over everything.

More negative is more advanced and -1.0 is a good setting. Little advanced over stock but not too much to where it gets more noisy.
