94 K2500 Burb with continuous Issues

Hello all. Want to say thanks in advance and have been doing my research on here before proceeding with this post for assistance. Great knowledge here! Any assistance will be appreciated.

Problem: Intermittent Stalls whether cold or hot. Surge/Fishbiting at 45 - 50 mph, happens when in limp mode. SEL trip and disappear at 65-70 mph happens when not in limp mode.

Demographics:
- Year: 1994
- Truck model and class: K2500, 8,600 GVWR
- Automatic or Standard: Automtic, rebuilt 11/25/2010.
- Mileage: 303000 miles
- Replaced parts and mods (IP, PMD, chip/PCM, TM, injectors etc): PMD replaced with #9 resistor, remote mounted to bumper on 05/23/2010. New PMD on IP replaced on 03/01/2011. Disabled using bumper.
- Model: Turbo-charged L65 "F"
- Ambient outside temp: 92F
- What fuel are you using? #2 Diesel
- What fuel additives are you using? Lucas
- Where are you located?: Slidell, LA

General Maintenance:
- Air filter: Currently doesn't have one, previous owner took it out, Working on getting one.
- Fuel filter: Bought from Oriellys Auto, Think BWD. Replaced today 6/24/2011
- CDR Valve?: Looks OEM
- Coolant – Uknown. Looks good. Replaced Cap at Firestore during maintenance along with thermostat. Had coolant leak 11/15/2010.
- Oil – Shell HD 15-40W. Just recently changed on 06/01/2011.
- Batteries – Matching Set from Walmart, dated 11/2009
- Battery cables and grounds (removed, cleaned and tightened): Removed all grounds and cleaned today.
- Glow Plugs - how many miles on them?: Unknown miles, replaced in January of this year. Got it from Oriellys Auto Parts. What type? Unknown
- Injectors - last changed at: Unknown

History:
- What were you doing when it happened?: Happens all the time regardless of what I am doing. Runs great for a while then trips. Sometimes it takes longer to crank.
- How was the truck running fine before this problem appeared?: Been like this since I bought it.
- Has this problem ever happened before?: Yes but more persistent.
- Describe any related factors that might be part of the problem: Replaced fuel lines from lift pump to IP. Had a leak.
- Have you checked for DTC (Diagnostic Trouble Codes)?: Yes. 17, 18, 19, 35, 36, 41, 43, 48, 54, 62, 78

Diagnostics:
1] Starting:
1a] Does the engine start?: Yes with OS unplugged and plugged.
1b] Does the engine crank over?: Yes.
1c] Are your batteries fully charged?: Yes.
1d] Have you removed, cleaned, and tightened all battery connections?: Yes.
1e] Does your Wait to Start light come on for a time?: Not on this model.
1f] Does your glow plug light come on?: Yes for 5 seconds.
1g] Do you see smoke from the exhaust while cranking?: Yes. What color?: Normal exhaust colored I think.
1h] Does the Service Engine Light Glow during Start/bulb check?: Yes.


2] Fuel Issues (if not running):
2a] Can you hear your lift pump working?: Yes. I just recently replaced this back in March 2011.
2b] Do you get a steady flow of fuel from the water drain (T-handle)?: Yes..
2c] Loosen the inlet line from the IP – is fuel coming out (Lift Pump on)?: Yes fuel is coming out.
2d] Loosen an injector line and crank the engine - does fuel come out?: Yes fuel comes out.
2e] Do you hear a vacuum (hiss) when you remove the fuel cap?: Yes.

3] PMD / IP Issues
3a] Location of PMD (on pump, on intake, remote mounted): Two new ones, using the one on bumper currently and has heat shield. Other on IP.
3b] Condition of Extension harness (if used) Vendor or Homemade?: Pensacola Diesel provided.
3c] Condition of grounds / ground wires to IP / PMD. Check Harness.: They are present, condition unknown.
3d] PMD Make: DTech on bumper, Standalyne on IP.
3e] PMD Age: Both are brand new.

4] Stalling Issues
4a] Do you experience stalling only when hot?: No.
4b] Does it stall under particular circumstances?: No.
4c] Does pouring cool water over the IP seem to help with restarting?: No
4d] Is stalling momentary (engine recovers)?: Sometimes, but SEL will trip
4e] Does SES light come on when engine hiccups?: Yes
4f] Does cruise control quit working when engine hiccups?: Yes
4g] Does re-starting seem to reset engine, restoring cruise and power?: Sometimes.
4h] Changes to dash lights or instruments during stalling events?: Only SEL

5] Running Issues
5a] On cold start, does upper rad hose get hard before engine warms up?: No.
5b] Any bubbles or oil film in coolant overflow tank?: No.
5c] Any white slime on oil surfaces (dipstick, CDR, oil Fill cap)?: No.
5d] Excessive white smoke from exhaust on cold start? Not that I am aware.
5e] Excessive black smoke from exhaust under hard acceleration? Sometimes

6] Performance Issues
6a] Do you have gauges (boost, EGT)?: Unkown.
6b] Maximum boost under load?: Unknown.
6c] Have you added a performance PCM (reflash) or Chip?: No.
6d] Have you changed exhaust / downpipe / crossover?: No.
6e] Wastegate – mechanical or vacuum (OEM)?: OEM. On my task to do list to replace.

List and describe any other factors which are not addressed in this checklist. We can’t see, hear, smell, or touch your truck, so the more descriptive information you can give, the more likely it is that somebody can assist you.

1. Sometimes when SEL is on I get sputtering on deceleration. This has just happened as of recently.
2. Coming back from Gulfport a couple of weeks ago, during deceleration letting foot off pedal truck kicked into acceleration without my foot on pedal.
3. Truck has randomly stalled while on highway. Not fun with semi behind you.
4. I have cleaned all grounds. I am starting to think I have a timing issue.
5. Did check for air in lines, none found. I did replace a cracked return fuel line on drivers side.
6. I did replace the Fuel Solenoid Shutoff on 5/20/2011. It was leaking. But I am not always hearing the click before starts. Wonder if it is getting stuck.

With all those codes, you would think it's "Ground/Wiring" related.

The first thing which is free would be to clean any and all grounds, that includes the battery and all ground straps. On the 94 I believe the fuse for the fuel shut off sol. is somewhere under the dash, it would be good to pull that fuse and shine the spades up.

Cleaned the grounds today and made sure they were good in tight. Still thinking it may be the sock.

I cleaned the fuse as well as the grounds. I sometimes hear the Fuel Shutoff Solenoid Pop and sometimes I don't. Wonder if it is stuck. I do have a fuel tank of gas as well, so would have to burn some of that off before attempting to change the sock from the previous post.
 
I just went out for the past hour, cleaned up fuses etc. Started the beast up and this is my new findings:

1. First Start, ran fine for 5 minutes and stalled. Codes, 17, 18, 35, 36
2, Second Start, ran for 45 minutes and stalled. Codes 17, 18, 35, 36, 48
3. Third Start ran fine and stalled after 10 minutes. Code 17, 18, 35, 36, 48.

Lift pump is working. When I replaced OPS, I noticed on instrument cluster that it leans more to right and slowly makes its way back down to 40 when warmed up. Don't know if I got a shotty OPS. Thanks guys for assistance. Just don't want to break bank even more with troubleshooting.
 
Don't know if the IP has ever been replaced. Will look further into it tomorrow to determine that. Just trying to resolve all issues before proceeding with replacing IP.
 
next time it stalls see if you can test fuel flow or better yet put on a fuel pressure guage and see what's going on when it stalls. also put a piece of clear tubing on the reurn coming out of the IP and check for bubbles
 
Pull the plunger on FSOS......and check the plug and harness on both sides.

I just found mine was all hacked up......But pull the plunger just to be 100% sure.
 
With it throwing so many codes I'd be suspicious of the ecm. That many things don't go wrong all at once. 35 and 36 codes are set by most any other code being set. I wouldn't trust the extension cable from Pensacola[china crap]
 
Plug PMD with heat sink back onto short IP harness. Temp eliminate the ext harness and see how she does.
 
I've heard "Pensacola" has had some "quality control issues" on "certain" products......It's not impossible that you have a bad extention harness/cable...........Just sayin'
 
Pensacolas "new" PMD's are used cleaned up ones in most cases. That said, PMD's usually don't throw all those codes although could be more than one issue as well. 78 is Boost which maybe a separate issue.
 
Ok thanks everyone. Will pull the plunger off the Fuel Solenoid to rule that out. Will stop by Harbor Freight get fuel gauge as well as clear line for return. Will keep posted on finding.
 
Haven't pulled plunger off yet, after new install of FSS it is now a pain to get off. Anyways, did fuel pressure test and clear line from return. No bubbles visible in return line. Fuel pressure read at .42 psi and stayed steady. When truck stalled it jumped to .5 before backing down. Also rechecked all fuses to ensure that none were blown and replaced with new ones.

Both PMD replacements have caused the same issue. Don't think that is it but I always stand to be corrected.

I may try to replace OPS again. Any recommendations from parts stores? Just trying to cool off again. It's already hot here at 10:36. Sweating like a pig lol.
 
AC Delco OPS, if you can find one in stock at the parts stores.

.42 psi? that's incredibly low, provided you aren't checking at the return line. Just for clarification, you should be checking pressure at the inlet of the IP. You should have at least 4-5 psi at idle.
