The MAF sets the upper limit of the fuel that can be delivered to the engine so it could cause smoke. Not that it has to cause smoke. I would find someone with a scantool and look to see if the MAF is reading and that it changes values with engine speed before I did anything.

Pin # 16 is battery voltage and pins # 4 and 5 are ground. Your truck doesn't have any CAN connections in the OBD II plug so do not worry about that.

It does not mean your injectors are bad but that the cylinder is low on power. Most of the time it's the injector but in your case with the coolant issues that too could be causing it. One cylinder is showing it's taking away fuel while the other is adding fuel.

If you not in a hurry a bar of Ivory soap works great. Tear the top of the soap bar box open about 1/2 way and toss it under the seat. It will take a few weeks but once it starts to smell like soap remove the bar from the truck. The soap smell goes away pretty quick once the bar is removed
